Buried penis

The “buried penis” is a clinical condition in which the penis does not extend fully outside, but is instead hidden, trapped, within the surrounding skin and adipose tissue. It falls under conditions recognized as false penile brevity, because in this case, unlike micropenis, the penile shaft has a normal length but appears visually reduced due to the advancement of the abdominal-penile angle.

This condition can be:

  • Congenital
  • Acquired: Although obesity is not the only cause of acquired buried penis in adults, which can also result from scarring due to excessive circumcision, loss of penile skin tissue due to lichen sclerosus, or, in rare cases, pelvic lymphedema, it remains one of the most relevant risk factors for the development of this condition.

The buried penis may be accompanied by various physical, medical, and especially psychological discomforts. The patient may experience chronic pain, difficulty urinating in an erect position, erectile dysfunction, and recurrent urinary tract infections. All of this can obviously lead to a low quality of life.

The diagnosis is essentially clinical. The physical examination should include a comprehensive analysis of the patient’s body shape and functional conditions. It is essential to perform an examination of the abdomen and the suprapubic area. Often, there is an accumulation of adipose tissue in the suprapubic region that can conceal the penis. Additionally, the genital-urinary assessment must be conducted with particular care, paying attention to the circumcision status, the condition of the meatus, and the appearance of the skin of the penis, scrotum, and suprapubic area, looking for signs of skin thickening, lymphedema, and dyschromia.

The treatment, if weight loss is insufficient, is surgical. There are different surgical techniques for correcting this condition, and the therapeutic choice depends on various factors such as the severity of the case, the underlying cause, and the patient’s preferences.

The buried penis is a condition that can cause significant discomfort and embarrassment for the patient, particularly in more intimate situations; often the result is isolation. For this reason, it is crucial to provide the patient with the right emotional support and reassure them that, although it is a complex condition, it can be treated with the right assistance.
